Notice of Data Breach – Life in Motion Family Wellness Center

Life in Motion Family Wellness Center is committed to protecting the privacy and security of our patients’ protected health information (PHI). This notice is to inform the public of a recent privacy incident involving unauthorized access and misappropriation of certain patient information. On July 22, 2025, Life in Motion discovered that patient information — including name, date of birth, telephone number, and address — had been accessed without authorization. Our investigation determined that the information was obtained by a former renter of Life in Motion Family Wellness Center office space and subsequently provided by her to a local physician for marketing purposes and to solicit patients.

We immediately took steps to address the situation:

• All system access reviewed and new layers of protection and safeguards put in place.
• The source of the breach was identified and secured.
• Federal, state and local agencies were notified in accordance with HIPAA breach reporting requirements.
• A police investigation is ongoing, and Life in Motion Family Wellness Center and Kathalene Keller Riney, NP are cooperating fully with law enforcement and regulatory agencies.

We take this matter extremely seriously and are implementing additional safeguards to further protect patient information. At this time, there is no evidence that the information was used for purposes other than the marketing and solicitation activity described above, but patients are encouraged to remain aware and report any suspicious contact.
